The petition must be filled in computer and printed out.
All information must be filled in correctly and completely.
Delete the dotted places and write the requested information in parentheses, then the relevant parenthesis explanations should be deleted.
Name, surname, student number, address and contact information should be written in the relevant places and the explanations should be deleted.
